Health Clinic Open Through Christmas Break

Dear D49 Families, SBHC open during D49 Christmas Break announcement

The school-based health center at Falcon Elementary School of Technology, operated by our partners at Peak Vista Community Health Centers, will remain open through the D49 Christmas break with the exception of Christmas Day (Dec. 25) and New Year's Day (Jan. 1, 2020). Clinic will be open 1/2 day Christmas Eve (Dec. 24) and New Year's Eve (Jan. 31). Flu shots are available!

The SBHC offers the following services: 

Services

  • Acute illness care and management 
  • Diagnosis and treatment of sudden, minor illness and injury
  • Immunizations
  • Physical exams
  • Well-Child Check-ups
  • Specialty and Behavioral Health Referrals 
  • Dental referrals to Peak Vista’s Family Health Center Care Management
Hours of Operation
  • Monday thru Friday, 8 a.m. – 4:30 p.m. year round, last appointment is at 4:00pm

** Peak Vista Community Health Centers provide basic health care to district students and their siblings, 0-21 years of age. All insurance plans are accepted, as well as uninsured patients. 

This health center has information for patients regarding insurance assistance and community resources.

  • Screening & application processing: Medicaid, CHP+, Colorado Indigent Care Program (CICP) and sliding fee scale program
